Work in partnership with the CSHCN medical teams in providing families with information and linking them to services as part of the care planning process
Provide one-on-one assistance to CSHCN families in navigating educational and medical systems by providing technical assistance, training, organizational tools, and assistance with the communication skills necessary to best advocate for their child.
Assist the CSHCN program by providing a family perspective on procedures, practices, and materials
Participate on advisory committees and attend trainings, conferences, and workshops to increase knowledge of available community resources
Facilitate Circle of Parents support groups in an effort to strengthen families through social connections and informal support systems
Assist families with transitions through the educational and medical systems.
Empower families to advocate for themselves and for their child in their communities
